This is the year-end adjustment (年末調整) form where you declare life insurance, earthquake insurance, 国民年金 you paid yourself and iDeCo, to pay less tax. You copy the amounts from your certificates. We apply the formulas and send back the official form filled in as a PDF.
Write the insurance company name and the type of insurance in Japanese, as shown on the certificate. Your マイナンバー is not requested and stays blank.
Only what you paid outside your salary this year: 国民年金, 国民健康保険 from before you joined the company, or a family member's part that you paid.
These are the amounts that go on the form. The deduction lowers the income that tax is calculated on. Your company does the adjustment in the December payslip.
The PDF arrives in your email a few minutes after payment (up to 15 minutes). Check your spam folder too.

It is one of the forms your company hands out in November for the year-end tax adjustment (年末調整). On it you declare what you paid for life insurance, earthquake insurance, pension plans and social insurance you paid yourself. If you do not hand it in, you lose the deduction in the adjustment and can only get it back by filing a tax return.
Life, medical and private pension insurance: up to ¥120,000 combined. Earthquake insurance: up to ¥50,000. 国民年金, 国民健康保険 and iDeCo: the full amount you paid. The formulas are printed on the form itself, and they are the ones we apply.
It is written on the certificate. In general, contracts made in 2012 or later are 新 (new) and earlier ones are 旧 (old). The formulas and limits are different for each.
